Upon arrival to your first dental appointment, you will be asked to fill out a few quick forms. Therefore we always ask that you arrive about 20 minutes early to allow for some extra time to get these forms filled out before we begin treatment.
To save time, you may print the New Patient Form and Financial Responsibility Forms by clicking the download link below. You can fill them out online and print them out or email them to us. Make sure you bring them with you for your first dentist appointment.
